I saw this app listed as free on the App Advice listing for today.
It's still free at this moment. Don't know about its accessibility
with VoiceOver but it won't cost you anything to try.

Another thought:
Using the built-in reminders feature of iOS could meet your need. I
use FantastiCal which makes setting up repeating reminders even
easier and more versatile.

Hello,

I think the app, Alarmed, Reminders and Timers would work perfect in this situation.
The difference in this reminder app and the Apple Reminder app is its flexibility in all of its features. I currently use this app for all of my reminders.

There are 3 features that stand out to me that would be great when reminding yourself to take medicine.

The Nag feature. With this feature, you can set it to keep reminding you in designated intervals about something you need to do. For example, say you have a reminder set for 12 noon to do something with the nag feature set to remind you again every 30 minutes in case you forget. If you are busy doing something when the reminder goes off at noon, and you intend on doing it when you get done, but by that time, you have forgotten about the task you were going to do. Then all of a sudden the nag feature goes off at 12:30, and you say, oh yeah, I need to do that! A great feature, saved me many times!

The Day Reminder Feature. The day reminder feature would be great to use with taking medicine. with the day reminder feature, you set a reminder to repeat a specific number of times within a 24 hour period. Now this feature is in addition of the normal repeat feature. And you can use this feature along with the nag me feature if needed.

Here is an example.
Say you have to take medicine 3 times a day, and you take it during breakfast, dinner, and before bed. How would you set this up?
You would create a reminder to go off at the first time of the day that you had to take the reminder. Lets say, 8 Am.
Since you take it every day, you would set it to repeat every day.
Then you would set the Day Reminder feature to repeat 3 times during the specific duration of time. Lets say, between 8 Am and 10 PM. That's 14 hours.
Now, in case you do not hear the original reminder, just for safety reasons, you set up the nag me feature to nag me every 5 minutes until you mark it complete.

So here is what would happen:
Your first reminder would go off at 8 AM, and that reminder would repeat every 5 minutes until you marked it done.
Then 7 hours later at 3 PM, the process would repeat for the second time to take your meds.
Then lastly, at 10 PM, the process would repeat for your final time to take your meds.

Now, one last point. What if you have several different medicines to take. No problem, you can create a separate reminder for each medicine, then even assign a different sound for each medicine.
